ACHD’s Chronic Disease Prevention Program works to improve the quality of life of Allegheny County residents. Based at the Hosanna House in Wilkinsburg, the program assists residents by enabling them to prevent, detect, delay and/or manage heart disease, high blood pressure, diabetes, cancer, and other chronic conditions.

In addition, the program tracks trends in illnesses and death in Allegheny County residents from these major diseases using information from death certificates, hospital discharge data, and the Allegheny County behavioral risk factor surveillance survey.

Serving as a resource to social service agencies, community organizations, employee wellness committees, and health care providers, the program provides information about evidence-based practices for screening, prevention behaviors, and best disease management guidelines for the prevalent chronic conditions. Moreover, the program partners with various wellness organizations and advocacy/awareness groups to boost public outreach and awareness about healthy eating, increased physical activity, and tobacco cessation.

As part of the collaboration with community partners, the Chronic Disease Prevention Program ascertains needs, identifies gaps in services, and engages in combined efforts to address systems change and policy development that promote healthier lifestyles necessary to decrease chronic disease risk factors.  To foster partnerships, our program’s staff members attend coalition and organization meetings, serve on advisory boards, and function in committees as needed.

Another focus of the Chronic Disease Prevention Program is linking and referring people without resources to needed health care services.  This is accomplished through updating and promoting  Underinsured and Uninsured?
